Fill them out and bring them with you to save time during registration.
Parents/Guardians must present the following documents when enrolling their children in the South-Western City School District:
- Lease, deed, or two current utility bills (gas, water, home phone, electric) in the parent/guardian’s name
- The student’s original birth certificate, hospital certificate, or baptismal certificate
- In the case of divorce or change of custody, a court document signed by a judge awarding custody to the parent/guardian who is registering the child
- The student’s immunization records
- If the student is new to the United States, proof of a negative TB test

How to Register
It is
necessary to provide certain documents when enrolling a
child in the South-Western City School district (see
below). Enrollment is open only to those residents who
reside within the South-Western City School District. To
enroll, go to your child's school. If you are not sure
which school to enroll your child in, please contact
Pupil Services at (614) 801-3056 or Communication
Services at (614) 801-3027.
VI)
A) PROOF OF RESIDENCY as
established by ONE of the following:
1) Signed lease (rent receipts are
NOT acceptable)
2) Signed deed or closing
statement
3) Two current utility bills
(phone, gas, water, electric) in the
parents' name at the enrolling
address
4) Completion of a verification of
residency form obtained at any school
building or the
District Service Center, 3805 Marlane
Dr., Grove City; (614) 801-3000
B) PROOF OF CUSTODY OR
GUARDIANSHIP granted by a judge
through a court of law, if there
has been a divorce or dissolution.
1) Divorce or dissolution
documents signed by a judge and recorded.
2) If custody or guardianship is
BEING CHANGED, we can enroll a
student for a maximum of 60
days, if we have in our possession
ONE of the following:
A) A letter from an attorney,
on company letterhead, indicating
that he/she represents
the enrolling adult and the necessary
paperwork has been
initiated in a court of law.
B) The enrolling adult
submits a receipt from Probate Court
indicating that an
application for guardianship has been filed.
3) Notarized statements and/or
powers of attorney are NOT
acceptable as proof of
custody.
C) BIRTH CERTIFICATE
or other evidence of birth as noted in board policy for each child being
enrolled. This includes: original birth certificate, baptismal certificate with seal, or hospital certificate with seal.
D) IMMUNIZATIONS
as required by law
1) D.P.T. (5)
2) Polio (4)
3) Measles, Mumps, Rubella (2)
4) Hepatitis B (3)
5) Varicella

II) STUDENTS OVER 18 YEARS OF
AGE who wish to establish
themselves as an independent adult
need to pick up the necessary
forms at the

III) ALL FOREIGN EXCHANGE STUDENTS
enrolling in our schools are subject to
the same rules and regulations as
United States citizens. No school
shall request proof of the following:
visa, passport, green card, I-94. |

IV) ALL FOREIGN EXCHANGE STUDENTS
enrolling through an
accredited foreign exchange program
will be approved by the high
school principal. |

V) ALL FOSTER CHILDREN in
the custody of a child welfare agency
must complete the Children's Services
form, in full, BEFORE the student is
enrolled in any of our schools. All
requested information must be
completed prior to enrollment, unless
specifically directed by District Service
Center personnel to the contrary. |

VI)
Students who have entered the United
States from a foreign country in
the past twelve (12) months must show
evidence of a tuberculosis skin test prior
to enrollment in the South-Western City
School District. If the tuberculosis skin
test is positive, then a negative x-ray is
required. |

Peer Model
Registration
If you are interested in having your three to five year old child become a peer model, and being taught in a classroom with disabled and non-disabled children, please call the Preschool Center at 801-8448 for office hours and enrollment information.
Applications are accepted throughout the school year. Children who are several months under the age of three and over the age of three are encouraged to fill out a peer model application. Peer model applications are dated the day they are received at the preschool and applications are reviewed in the order in which they are received. However, only children who are or will be three years old at the time of the peer observation will be contacted to participate when openings become available. |

When enrolling your child, please make sure to bring the following
documents:
- Child's original birth certificate (If you don't have the original, you can get another one at the Bureau of Vital Statistics in downtown Columbus located at 220 Parsons Ave. 614-645-7331)
- Child's up-to-date Shot Record
- Verification of Residence (at least one)
- Two current utility bills
- Current Lease
- Settlement Statement
- Verification of Income (at least one)
- Last pay stub
- OWF statement/FCDJFS benefit letter
- W-2 or 1040
- Custody Papers, if applicable
- IEP, if applicable

Parents should call the elementary school in their attendance area to register their child.
To enter kindergarten, a child must be five-years-old by
September 30. Parents must bring the following items
(both originals and copies) to registration:
- Child's birth certificate
- Immunization records
- Child's Social Security Number (optional)
- *Proof of residence
- Custody papers (if appropriate)
*May be
a lease, deed, closing statement, or two current,
different utility bills with custodial parents' name and
address (cable bills not accepted). If there has been a
divorce, please bring proof of custody (final court
decree signed by a judge). It is necessary to have all
documentation in order to process registration in a
timely manner.

School Fees
The annual $20
student instructional fee covers the cost of your child's consumable
school supplies - pencils,
paper, glue, scissors, folders, etc. Therefore, South-Western does
not post school supply in area stores.
Teachers may ask
for some additional items such as tissues, paper towels, zip lock
bags, etc., but should not
be asking for donations of everyday school supplies.
Also note that at the middle school level it may be
necessary to purchase combination locks for lockers, where
required. These locks will be available at the school price. At the
high school level please expect additional fees
for workbooks, labs, fine arts, band, etc.
